Argyll House doctor's surgery in Ryde is to close, the Isle of Wight's Integrated Care Board (ICB) has revealed.
Health bosses have received a notice of closure from the surgery's managers - who have handed in their contract to deliver general medical services.
The news has been shared on the surgery's website.
In the meantime, Argyll House is open as normal for registered patients, who are being told to take no action.
Argyll House Surgery says it cares for 'thousands of patients in Ryde, Wootton Bridge and the surrounding villages' and has been open for over 25 years.
Its team includes four GPs, two advanced nurse practitioners, two practice nurses, and two healthcare assistants, as well as a pharmacist, practice manager, reception and secretarial staff.
Its website says it runs a full range of general practice services, with special clinics for babies, pregnant women and patients with certain conditions.
Darren Cattell, acting place director for Isle of Wight at the Hampshire and Isle of Wight ICB, said: "We will keep patients informed of developments as we work with the practice and support their staff through this process.
"Our priority will be to ensure patients continue to have access to high quality primary care services.
"For the moment, Argyll House remains open and it's very much business as usual for patients who need appointments or want to access other services."
The next steps are being considered and patients will be contacted in time, the ICB said.
